The mace is a glorified club and used like a club. It is NOT the spiked ball with chain and club which is actually called a Mourning Star. The mace has the spiked ball directly on the club.

Now the reason the mace was used in combat is related to religion. Fighting priests who were under vows could not shed blood. The mace was designed to crush skulls and break bones and was an acceptable weapon for clergy. Yeah, sometimes some blood got spilt, but they justified that some way.

The mourning star was not widely used because, as wicked as it looks, it’s almost as deadly to the person using it as the person they are attacking. Because it looks so deadly, though, Hollywood loves it.

The mourning star used in the battle between Eowyn and the Witch King of the Nazgul in Return of the King was all CGI. Too dangerous to have a real one in use.
